Job Description:
A company is looking for a Case Manager.
Key Responsibilities
Coordinate care, resources, and services for members to achieve
optimal clinical and resource outcomes
Develop and implement member-centric care plans, addressing
barriers to achieving health goals
Monitor and evaluate member progress, making necessary adjustments
to care plans as needed
Required Qualifications
RN, RD, or LCSW with 3 years of clinical and/or case management
experience required
Master's in Social Work with 5 years of related experience may be
considered for some roles
Valid and applicable clinical license (NC or compact multi-state
licensure) required
For some roles, additional specialty certification (i.e., CCM,
CDCES) may be required
Minimum of 3 years of clinical experience for Behavioral Health
specific roles with applicable licensure
